At the absolute minimum 4-6 weeks of supervised rehabilitation is a good idea. Conservative treatment should include a thorough lower extremity evaluation making sure to look at hip and ankle mechanics and function. Strengthening of the hips, core, quads, hamstrings, and really the entire lower extremity.

A lateral release for patella femoral syndrome should be the last resort after failed conservative rehabilitation. I have seen countless patients who were given the option of surgery or rehab and who chose rehab and had great results - so it is definitely worth a try.
