Ice first...
by: Bart - SII

If you have an acute injury, ice is recommended for the first several days to help control inflammation and secondary cell death. Applications of 15-20 minutes several times a day is standard. As far as heat, most will recommend not applying heat until after the acute swelling has subsided, usually 7-10 days after the injury. This becomes somewhat of a personal preference.

I recommend always following heat with ice during the acute inflammation stage.
