Swelling right knee with fluid

I had my right knee swollen for 2 month, X-Ray and MRI show no tear, Orthopedic doctor aspirated the fluid and injected cortisone which helped a lot ( 3 times so far). I have had no injury or trauma to my knee. Last time when I hit the couch (not hard) it swelled.

Ice and therapy did not help so doctor had to drain synovial fluid and injected cortisone. Doctor have no answer.

QUSTION: How often (and what negative side effect) can synovial fluid be removed from my knee? Lab showed normal fluid result.

The primary concern is what is causing the fluid buildup - have they done blood work looking for systemic causes for the swelling?

Aspirating the knee is likely going to reduce the swelling, but it won't fix whatever is causing the problem.
