What Does This Mean? Will I Need Surgery?

hi i have abnormal cartilage within femoral joint in the knee severe on patellar side? grade 4 cartilage loss subcortical cyst and oa what does this mean and what type of surgery would i be given

Arthroscopic surgery to "clean up" the knee joint

You have some degeneration to the cartilage of your knee, the undersurface of the patella and the femoral groove, from what you describe in your post. Grade 4 loss refers to the severity of the cartilage degeneration - this is called chondromalacia. The subcortical cyst is probably an accumulation of fluid just beneath the surface of the bone - likely due to the cartilage loss. And osteoarthritis is a general term describing all of your other findings.

Surgery to clean up and smooth out the joints is probably your next option. Grade 4 chondromalacia means that you have lost cartilage and exposed the underlying bone - sometimes this is treated by simply smoothing out the surrounding cartilage if the area of exposure is small - if it is a large area it can be treated with microfracture - basically very small holes are drilled in the bone to cause it to bleed, and then the body will form scar tissue on top of the bone where the cartilage used to be.

Arthroscopic surgery is the best way to treat this, and what is actually done will be dependent on what your surgeon sees when he actually gets into the knee joint.
